What to send

  • The participant's first name and suburb
  • The supports wanted, and which days and times
  • How the plan is managed
  • Anything a support worker needs to know to be safe on a first visit, like a dog, stairs or a key safe
  • How the participant would like us to make contact

What not to send yet

No plans, reports, diagnoses or NDIS numbers through the form, please. We'll ask for what we need, with the participant's consent, once they've decided to go ahead, and we'll ask for it securely.

Want the background first? Read what a support coordinator does, from a provider's side.

Who should we contact first, and how? (required)

Your privacy

We use what you send only to reply to you and arrange support. Only our office team reads it. You can ask to see or correct what we hold at any time. Please don't include health details, a diagnosis or your NDIS number here: we'll ask for what we need when we talk. Our privacy policy.

Demo form: nothing is sent. Submit it to see the thank-you page.

Questions from support coordinators

Can't see your question? Ring and ask a person. A team leader answers, Monday to Friday.

How do I refer a participant to Glenrock Support?

Use the referral form on this page, ring the office, or email us. We need only the basics to start: the participant's first name, suburb, the supports and days wanted, and how the plan is managed. We reply within 2 business days.

Do you accept NDIA-managed participants?

Yes. We are a registered NDIS provider, so we can work with NDIA-managed, plan-managed and self-managed participants. Our registration number is on every page.

What if you don't have capacity?

We'll tell you on the first call, with a realistic date, rather than accept a referral we can't staff. If we can't help in a reasonable time, we'll say so plainly so you can try another provider.

Will the participant meet the support workers first?

Yes, always. A team leader brings one or two support workers to a free meet-and-greet before anything is signed, and the participant chooses. You're welcome to join if the participant would like you there.

What reports do you provide to coordinators?

With the participant's consent, we send a short summary at the end of the first month and before each plan reassessment: hours delivered against the service agreement, what's working, and anything the participant wants changed.

Do you provide support coordination yourselves?

No. We don't offer support coordination or plan management. Keeping them separate means there's no conflict between who coordinates the supports and who delivers them.
